Requirements for Report of Marriage

  1. Thai Marriage Certificate and Thai Marriage Registration Certificate issued by the Thai District Office that officiated the marriage. Both the Thai Marriage Certificate and Thai Marriage Registration Certificate must be translated into English and certified by the Thai Ministry of Foreign Affairs;
  2. Original Passports of the spouses and photocopies of their passport photo pages (Note: The Embassy does not provide photocopying service.);
  3. Four (4) sets of Report of Marriage form signed by the spouse/s before an Embassy official;
  4. Payment of processing fee for Report of Marriage.

Reminder: Documents issued in the Philippines, including PSA-issued documents (e.g. Certificates of Birth, Marriage, Death, and No Marriage Record / Court Documents etc.), should have a Philippine apostille before they can be used as a supporting document for consular services.
