The new passport application forms come in two pages (front and reverse).

At the Embassy in Bangkok, we use the following color coded forms:

WHITE for ADULT applicants renewing their e-Passports (regular renewal).

PINK for ALL MINORS (below 18 y.o.) whether first time or renewing their e-Passports.

YELLOW for ALL OTHERS APPLICANTS: lost or damaged passports; changing to married surname; those using old passports other than e-Passports; other cases.
